Androgenetic Alopecia, also known as Male-Pattern Baldness, is the most common cause of hair loss. About 50% of men will experience this form of hair loss by the time they’re 50 years old. As far as the ages in which baldness can occur, it can start as early as 12 and as late as 45. Though essentially benign, there are psychosocial effects that male-pattern baldness can cause. A major determinant of AGA concerns testosterone (T), more specifically the enzymes that affect androgen metabolism. In men, testosterone plays a key role in the development of male reproductive tissues such as the testis and prostate as well as promoting secondary sexual characteristics such as increased muscle, bone mass, and the growth of body hair.
Out of the approximate 5 million total hair follicles on the human body, roughly 100,000 are on the scalp. The medical term for hair loss is known as Alopecia, and can be caused by varying factors. Some factors that can cause (temporary or permanent) hair loss include:
- Medical Conditions: Thyroid Disease, Alopecia Areata, Ringworm (scalp infection), certain medications for cancer, high blood pressure, arthritis, depression, heart problems.
- Psychological/Emotional factors: Trichotillomania (Compulsion to pulling out one’s own hair). Emotional factors include a death in the family, extreme weight loss, a high fever, ect.
- Physical factors: Traction hair loss can be due to hairstyles that put pressure on the follicles by pulling the hair back very tightly. A diet lacking in essential vitamins can also contribute to hair loss.
Finasteride was originally used to treat men with benign prostatic hyperplasia (BPH), but a side factor of the medication included combating male pattern hair loss, also called androgenetic alopecia. It may be used alone or in combination with other medicines such as an alpha-blocker to treat BPH. For men with hair loss, finasteride will increase the number of scalp hairs but will not increase the amount of body hair.
Originally created to treat high blood pressure, soon after that it was discovered that Minoxidil had properties that promote hair growth in patients. It is effective in helping promote hair growth in people with androgenic alopecia regardless of sex. About 40% of men experience hair regrowth after 3–6 months. Minoxidil must be used indefinitely for continued support of existing hair follicles and the maintenance of any experienced hair regrowth. The mechanism by which minoxidil promotes hair growth is not fully understood. Minoxidil is a potassium channel opener, causing hyperpolarization of cell membranes. Theoretically, by widening blood vessels and opening potassium channels, it allows more oxygen, blood, and nutrients to the follicles. This may cause follicles in the telogen phase to shed, which are then replaced by thicker hairs in a new anagen phase.
- During the first 1-2 months, you may not see much improvement, however don’t fret! It’s all part of the process. Around the 4-5 week mark, slight shedding can occur, but this is completely normal and temporary, as well as a good prognostic sign.
- At 3-6 months, the majority of progress will be seen. During this period, you will see hair loss slow down or even reverse. Hair growth will only improve from this point on! Be sure to take pictures before starting the topical hair boost and after to track how far your hair growth has come.
